We are asking all volunteers to complete this simple form letting us know as much information you have about the facility you are now serving at. We are also interested in collecting names of people who have not yet begun to serve but who are interested in carrying the message to the incarcerated alcoholic who is unable to get to an outside meeting of Alcoholics Anonymous.
